Don’t wait until the last minute! Getting your REAL ID early will ensure you’re prepared, and we’re here to support you every step of the way.
Please remind your youth/young adults to bring the following:
· $11 Reduce FEE form (Please contact your ILP Coordinator for the reduce fee form to ensure rate). Please bring cash, credit/debit card or check for payment
· Bring ORIGINAL documents (birth certificate, passport, permanent resident card etc.)
· Bring TWO forms of California residency in the caregiver or youth’s name (home utility bill, cellular bill, bank statement etc.)
· Bring your Agency-Group Home Agreement, Placement-Agency Foster Family Agreement or Foster-Parent Agreement must be typed not hand written
· Bring your Social Security Card or number
